One of the limitations of our study is that there are no standardized reference ranges for ROTEM and TEG. Nielsen 27 reported a 3-fold shorter CT compared with the TEG R time with a 7% higher TEG 6s functional fibrinogen level was compared with plasma fibrinogen concentration, measured using the Clauss method. Correlations were classified as weak (Spearman correlation coefficient 0.20 to 0.39), moderate (0.40 to 0.59), strong (0.60 to 0.79) or very strong (≥0.80). TEG® VERSUS ROTEM® Comparison. Two commercial types of viscoelastic tests are available: thromboelastography =TEG® (developed in 1948, now produced in the USA) and rotational thromboelastogram = ROTEM® (from Germany) differences in diagnostic nomenclature for identical parameters between the two Interpretation of TEG Factsheet v1 Final March 2013.doc Page 1 of 2 Interpretation of TEG/ROTEM Factsheet 3 Area of Application The interpretation of the traces produced by the thromboelastography devices is vital for the management of coagulopathy and the corresponding appropriate use of blood components/therapies.

When comparing TEG to ROTEM, with additional Two of the most commonly used viscoelastic hemostatic assays, thromboelastometry (ROTEM) and thrombelastography (TEG), have proven to decrease allogenic blood transfusions with cost reduction and possibly decrease mortality and morbidity in cardiac, trauma, and other bleeding patients. This article compares the two devices to provide guidance on the selection and use of these monitoring systems. Thromboelastography® (TEG®) and Thromboelastometry (ROTEM®) provide global information on the dynamics of clot development, stabilisation and dissolution that reflect in vivo haemostasis. Although TE has not been subjected to the same evaluation processes as conventional haemostatic tests, its use as a POCT monitor in complex major surgery has been shown to significantly reduce the use of blood component therapy and overall blood loss. Compared with transfusion guided by any method, TEG or ROTEM seemed to reduce overall mortality (7.4% versus 3.9%; risk ratio (RR) 0.52, 95% CI 0.28 to 0.95; I 2 = 0%, 8 studies, 717 participants, low quality of evidence) but only eight trials provided data on mortality, and two were zero event trials.

Main outcome measures: TEG 6s assays kaolin (CK), RapidTEG (CRT), kaolin with heparinase (CKH) and functional fibrinogen were compared with ROTEM Sigma assays INTEM, EXTEM, HEPTEM and FIBTEM. TEG 6s functional fibrinogen level was compared with plasma fibrinogen concentration, measured using the Clauss method.

Irrespective of which bit is rotating, some impediment to the rotation develops as the blood clots.

In both methods, whole blood is inserted into a cup, which must be pre‐warmed to 37°C. A pin that is connected to a torsion wire in the TEG system or to an optical detector in the ROTEM system is then immersed in the blood.
